In this vital role, you will provide high-quality patient care by assisting with daily activities, monitoring vital signs, and supporting medical staff in delivering exceptional care across various settings. Your energetic approach and attention to detail will ensure our patients feel safe, comfortable, and well-cared for throughout their recovery or ongoing care journey. This paid position offers an exciting opportunity to make a meaningful difference in the lives of individuals of all ages and health conditions. Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Asssig with enrich activitie, transportation and appointments. Housekeeping Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately. Support medication administration under supervision, ensuring proper documentation and adherence to HIPAA regulations Provide specialized care for individuals with, TBI, dementia or Alzheimer's disease, including memory care techniques and behavioral health support Assist with medical procedures such as catheter care, phlebotomy preparation, and clean and aseptic techniques in long-term care facilities Collaborate with interdisciplinary teams. Maintain a clean and safe environment by following infection control protocols and using appropriate personal protective equipment (PPE) Requirements Valid CNA certification with current state licensure Experience working in diverse healthcare environments including hospitals, nursing homes, assisted living, or home care settings Familiarity with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or similar platforms Knowledge of medical terminology, anatomy, physiology, and aseptic techniques Ability to perform physical tasks including lifting patients with Hoyer lifts and assisting with mobility needs Strong communication skills to effectively document patient information and collaborate with healthcare teams Experience caring for individuals with developmental disabilities or behavioral health needs is a plus Join us in delivering compassionate care that transforms lives!
