CSL Staffing is seeking a compassionate and motivated Lead Care Manager (LCM) for an excellent Temp-to-Hire opportunity with a growing healthcare organization serving individuals with complex medical and behavioral health needs.

Cerritos, CA Schedule:
Monday-Friday, 8:30 AM - 5:00
PM Pay:
$25.00 - $26.00 per hour
Position Type :
Temp to Hire Bilingual Spanish/English Preferred What You'll Do As a Lead Care Manager, you will serve as a key resource for members by helping them access healthcare services, coordinate care, and supporting them in achieving their health goals.
Responsibilities include:
Conducting member assessments and screenings Developing and updating individualized care plans Coordinating services with providers, community resources, and family members Monitoring member progress and documenting interactions Assisting members with overcoming barriers to care Providing compassionate support and advocacy Maintaining accurate and timely documentation Meeting with members in the community as needed What We're Looking For High School Diploma or GED required Valid California Driver's License, reliable vehicle, and current auto insurance Minimum 1 year of experience in Case Management, Care Coordination, Member Services, Patient Care Coordination, Customer Service, Call Center Support, Behavioral Health or Healthcare Support Services Strong communication and interpersonal skills Ability to manage multiple priorities in a fast-paced environment Comfortable using computers, email, and electronic documentation systems Bilingual Spanish/English preferred Field-Based Responsibilities This position includes community-based outreach and member engagement. Lead Care Managers are expected to meet with assigned members in person and coordinate visits within designated geographic areas whenever possible. Visits may take place at clinics, members' homes, community locations, or other approved settings. Why Join?
