  • Full-time Overview We are seeking a dedicated and compassionate Lead Care Manager to join our healthcare team.
Will serve as the primary point of contact for the Member, Member's family, Authorized Representative (AR), caregiver, other authorized support person(s) as appropriate, and the multidisciplinary care team providing care to the Member. The Lead Care Manager will develop a timely comprehensive Care Management Plan with input from a multidisciplinary care team, as well as the member, to ensure a whole-person approach is taken in identifying gaps in treatment or gaps in available and needed services.

AREAS OF RESPONSIBILITIES
1. Responsible for coordinating with those individuals and/or entities to ensure a seamless experience for the Member and non-duplication of services 2. Engage eligible Members 3. Oversee provision of ECM services and implementation of the Care Plan 4. Offer services where the Member lives, seeks care, or finds most easily accessible and within MCP guidelines 5. Connect Member to other social services and supports the Member may need, including transportation 6. Advocate on behalf of Members with health care professionals 7. Use motivational interviewing, trauma
  • informed care, and harm-reduction approaches 8. Coordinate with hospital staff on discharge plan 9. Accompany Member to office visits, as needed and according to MCP guidelines 10. Monitor treatment adherence (including medication) 11. Provide health promotion and self

Processes all inquiries in a timely manner and maintains tracking documentation. Meets with family members, or referring agency, and determines appropriate program. 13. Provides care management services to own caseload of patients in line with ECM guidelines and recommended services. 14. Collaborate with ECM Member and members of their care team to develop a person-centered Individualized Plan of Care (IPC) 15. Provides transitional care plan and graduation/discharge planning for all ECM Members. 16. Work with staff from discharging facilities, where applicable, to ensure timely and smooth transitions between different levels of care 17. Participate in supervision/check-ins with LCSW Clinical consultant and RN Clinical Consultant at a minimum of once a month 18. Identify and coordinate with ECM Member's primary care, CS service providers, behavioral health providers, specialist, community agencies, and other entities providing some level of care to ECM members. 19. Identifies and collaborates with ECM Member's social support system (family, authorized representative, care giver, emergency contact, neighbor etc.) 20. Provides signed and dated documentation for all services performed on the day the service is provided. 21. Active participant in the ECM Multi-Disciplinary Team 22. Active participant in all-staff and in-service meetings. Requirements may require outside training. Provide training for in-services as assigned. 23. Works as an integral participant of a management team concept. Maintains effective relationships with staff geared to the building and maintenance of a cohesive team. 24. Maintains all agency, licensing, contracts and administrative requirements for documentation as required for client services and administrative needs including raw data collections and tabulations and reporting requirements. 25. Assist in developing and providing educational and training programs for caregivers and professionals. Assisting in providing fact sheets and other information materials. 26. Actively identifies and refers potential members for ECM benefit 27. Conduct in-person visits with ECM member and members of their care team, including outside entities providing some level of care to ECM member.
MINIMUM QUALIFICATIONS
At least two years case management experience or lived experience with the PoF LCM will be assigned
  • A bachelor's degree in a healthcare field is preferred, but not required
CONDITIONS OF EMPLOYMENT
Physical clearance for reaching, bending, stooping, crouching, kneeling and grasping; have full range of motion; ability to lift 50 lbs; need to lift and transfer ECM Members; T.B. clearance; must maintain current first aid and CPR; must have reliable private vehicle, proof of CA drivers license and maintain proof of current vehicle insurance.
