Job Overview Cloud Recruiter LLC is seeking an experienced and compassionate Enhanced Care Management (ECM) Care Manager to support participants enrolled in Soul Housing's Enhanced Care Management program. The ECM Care Manager is a participant-facing position responsible for helping Medi-Cal members navigate healthcare, behavioral health, housing, benefits, and other community-based services. The ideal candidate is organized, resourceful, comfortable working independently in the field, and experienced in supporting individuals with complex medical and social needs. This position manages an assigned caseload, develops individualized care plans, coordinates services across multiple providers and community partners, and works directly with participants to identify and overcome barriers to care. This is not a remote position. Regular fieldwork at Soul Housing facilities and community locations throughout Los Angeles County is required. Responsibilities As an ECM Care Manager, you will: Conduct participant outreach, enrollment, and ongoing engagement. Complete comprehensive assessments and develop individualized care plans. Coordinate medical, behavioral health, substance use, housing, and social services. Schedule appointments, arrange transportation, complete referrals, and confirm participant follow-up. Support participants transitioning from hospitals, emergency departments, treatment programs, incarceration, and other institutional settings. Connect participants with housing, food, public benefits, identification, employment, legal services, and other community resources. Communicate and coordinate with healthcare providers, Managed Care Plans, hospitals, pharmacies, housing providers, and other members of the participant's care team. Identify potential safety concerns or crises and follow established reporting and escalation procedures. Maintain accurate and timely documentation to support ECM services, encounter reporting, audits, and billing requirements. Track participant progress, maintain required contact schedules, and effectively manage assigned caseload expectations. Participate in team meetings, case conferences, supervision, training, and quality-improvement activities. Maintain participant confidentiality, appropriate professional boundaries, and compliance with HIPAA and program requirements. Required Qualifications High school diploma or equivalent. At least 2 years of experience in case management, care coordination, healthcare navigation, homeless services, behavioral health, social services, or a related field. Experience working with individuals with complex medical, behavioral health, housing, or social-service needs. Strong communication, organization, documentation, and participant-engagement skills. Ability to independently manage a caseload and competing priorities. Proficiency with electronic case-management systems, email, and standard office software. Valid California driver's license and reliable transportation for field-based assignments. Preferred Qualifications Candidates with one or more of the following are strongly encouraged to apply: Bachelor's degree in Social Work, Psychology, Sociology, Public Health, Human Services, or a related field. Experience with Enhanced Care Management (ECM), CalAIM, Medi-Cal, or Managed Care Plans . Experience serving people experiencing homelessness, chronic health conditions, behavioral health conditions, or substance use disorders. Knowledge of Los Angeles County healthcare, housing, behavioral health, and social-service resources. Training in motivational interviewing, trauma-informed care, crisis intervention, or harm reduction. Community Health Worker certification or a similar credential. Bilingual English/Spanish or another language commonly spoken by the population served.

Location:
Los Angeles County, CA Regular travel throughout Los Angeles County is required. Work may take place in office, residential, healthcare, and community settings. Occasional schedule flexibility may be required based on participant and program needs.
