We are currently seeking a passionate and dedicated Care Manager to join our dynamic team at our Denver Tech Center location . About the Role As a Care Manager, you will play a vital role in coordinating compassionate, client-centered support within our collaborative treatment environment. You will work closely with clients, clinical teams, and community partners to ensure seamless care navigation and positive outcomes.
Essential Functions:
Provide comprehensive care management services to clients in need Conduct assessments to identify client needs and develop individualized care plans Utilize motivational interviewing techniques to engage clients in their care Coordinate with external agencies and resources to support client needs Offer addiction counseling and behavioral health support as needed Supervise and mentor staff members to ensure quality service delivery Assist clients in navigating Medicare benefits and entitlements Implement crisis intervention strategies when necessary Oversee workforce development initiatives for client empowerment Provide guidance on Section 8 housing options for eligible individuals
Education and Experience Requirements:
High School Diploma or GED Proficiency in motivational interviewing techniques Knowledge of addiction counseling principles Strong background in behavioral health services Ability to handle crisis intervention situations effectively Familiarity with Medicare regulations and benefits Experience in workforce development programs Proficient in crisis management strategies Understanding of Section 8 housing programs This position offers a rewarding opportunity to make a positive impact on the lives of those in need.
