(CHNCT) is currently seeking three (3) Intensive Care Managers - Rural Health for the Rural Health Transformation (RHT) Program. Qualified applicants should reside in one of the following areas: Litchfield County, CT Windham County, CT New London County, CT The Rural Health Transformation (RHT) Program is a federal grant that empowers states to strengthen rural communities across America by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem.
Primary Responsibilities:
Under the direction of the Care Management (CM) Manager of Rural Health Transformation, provide care coordination support for eligible HUSKY Health members residing in rural areas. Conduct targeted outreach to rural members to deliver community-based, person-centered care management services that address chronic conditions and complex health-related needs. Collaborate with CHNCT's clinical team, provider network, DSS and the community to create a process of assessment, planning and facilitation to meet the member's heath care needs across the continuum of care. Assist the member by functioning as an advocate for care preferences and service options by communicating effectively with other members of the healthcare team and by utilizing available resources to promote self-efficacy and achieve high-quality, cost-effective health outcomes.
Tasks Performed:
Conducts assessment(s) to identify member needs and develops individualized care plans that address opportunities, goals and planned interventions. Monitors and evaluates the effectiveness of the care plan and modifies as necessary. Provides education and care coordination to support members with managing their health needs and accessing appropriate services and resources. Conducts face-to-face visits, which may occur at the member's home, at a provider's office, or at various community settings. Presents at virtual member-facing learning groups on program-related topics. Participates in interdisciplinary case rounds/collaborative meetings as appropriate with other members of the Rural Health Transformation team. Orients new staff as requested. Performs other duties as assigned.
Essential Functions:
Engages with members through telephonic, videoconferencing and face-to-face outreach; and meets with providers and community organizations to coordinate care. Maintains knowledge of covered benefits for CT Medicaid, HUSKY Health programs.
Desired Education:
Two to four years post-secondary schooling required
Desired Degree:
Associate's degree, Bachelor's degree preferred

Nursing (required)
Desired Job Experience:
2 years direct clinical patient care or comparable experience is required. Behavioral health experience and knowledge of case management are beneficial.
Other Qualifications:
Preferably live in one of the designated geographic locations in need. Requires excellent verbal and written communication skills. Good time management and organizational skills along with the ability to manage continually changing priorities. Strong computer and keyboard skills. Ability to exercise independent and sound judgment in decision making. Managed care and case management experience preferred. CT RN License must be current and unrestricted. CCM or other URAC approved certification preferred. Must hold a valid and unrestricted motor vehicle operator's license and have reliable transportation.
