  • Free Online Resiliency Program. Are you ready to make a difference? As a Case Management Professional in the role Independent Living Coach, you will be responsible for:
  • Conducting psychosocial assessments and determining eligibility for services.
  • Developing and implementing individualized Treatment/Life Plans.
  • Providing ongoing supportive counseling to prevent relapses, psychiatric hospitalization, and incarceration.
  • Facilitating participant intake, program development, referrals, and discharge processes.
  • Offering coaching and counseling in areas such as personal and social adjustment, work training, and productivity.
  • Maintaining detailed case records and adhering to all documentation requirements.
  • Monitoring participants' progress and participating in case review meetings with stakeholders.
  • Positions will be in the office and/or onsite with participants.

Veteran Program:
Grant Per Diem (GPD) housing up to 80 homeless veterans transitioning to permanent housing. Must have Bachelor's Degree in human services and 1 years' experience working with adult population. Possess initiative and the ability to work effectively without close daily supervision. Help us realize our vision as we "strive to provide premier human services and experiences with superior results for individuals and families in need". Hourly pay rate $22.00 to $23.00 based on highest level of education. Don't wait. Apply today.
