• Conducting psychosocial assessments and determining eligibility for services.
• Developing and implementing individualized Treatment/Life Plans.
• Providing ongoing supportive counseling to prevent relapses, psychiatric hospitalization, and incarceration.
• Facilitating participant intake, program development, referrals, and discharge processes.
• Offering coaching and counseling in areas such as personal and social adjustment, work training, and productivity.
• Maintaining detailed case records and adhering to all documentation requirements.
• Monitoring participants' progress and participating in case review meetings with stakeholders.
• Positions will be in the office and/or onsite with participants.

Veteran Program:

Grant Per Diem (GPD) housing up to 80 homeless veterans transitioning to permanent housing. Must have Bachelor's Degree in human services and 1 years' experience working with adult population. Possess initiative and the ability to work effectively without close daily supervision.
