Job Description

Aging Life Care Manager- for immediate hire, FT & PT positions available. We are seeking a compassionate, organized, and resourceful Aging Life Care Manager to support clients in accessing services, achieving goals, and improving overall well-being. The Aging Life Care Manager will coordinate care plans, connect clients to community resources, advocate on behalf of clients, and maintain accurate documentation while building strong relationships with individuals, families, and service providers. Key Responsibilities Conduct client assessments and identify needs, strengths, and goals Develop and monitor individualized care/service plans Coordinate referrals to healthcare, housing, transportation, financial assistance, mental health, and community support services Maintain regular contact with clients to monitor progress and provide ongoing support Advocate for clients and assist with navigating complex systems and services Document all client interactions, case notes, and outcomes in the client management system Collaborate with internal staff, community partners, healthcare providers, and family members Assist clients during times of crisis and help identify appropriate interventions Ensure compliance with agency policies, confidentiality standards, and program requirements Participate in staff meetings, training, and community outreach activities Preferred Skills Knowledge of community resources and social service systems Experience with seniors, individuals with disabilities, behavioral health, or healthcare coordination Ability to work independently while also collaborating as part of a team
