A Nursing Home or Home Health Administrator manages staff and operations at a long term care facility. Oversees admission and care of residents, staff hiring, billing and finances, compliance with regulatory requirements and building maintenance. Maintains professional licensing requirements. Manages staff, recordkeeping, billing, finances, and other administrative work for a home health care program, where aides assist clients with personal, household and some health-related services in their homes.
The Care Manager for Central Intake will provide advocacy, coordination, and continuity of evidence-based care for individuals and families mental/behavioral crisis within our Intake Central unit. The Care manager will facilitate through a collaborative process of triage, assessment, planning, and communication in crisis management and care. Participates as a member of a multidisciplinary treatment team. Reports to the unit/department supervisor.

QUALIFICATIONS
Bachelor's degree in a Human Services-related discipline and at least 1 year of experience in the mental health field. Ability to read, analyze, and interpret clinical records, procedures, or governmental regulations. Ability to write reports and document clinical activities and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders.
FUNCTIONAL RESPONSIBILITIES INCLUDE
: Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements. Complete all clinical and administrative documentation based on established timelines as per program requirements. Communicate effectively and maintain cooperative relationships with internal and external resources. Develop service plans, treatment plans, and/or aftercare plans with clear goals and objectives as per program requirements. Maintain efficient workflow management and organization as per program requirements. Maintain proficient quality of clinical documentation as per program requirements. Other duties as assigned or needed as per program requirements.
PHYSICAL DEMANDS
Abilities in reaching, bending, talking, sitting, carrying, standing, grasping, hand-eye dexterity, ability to hear, ability to read and write, and the ability to remain calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch, or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 50 pounds of dead weight alone. May be required to facilitate presentations while standing or sitting.
WORK ENVIRONMENT
A secure facility that provides an inpatient treatment program for those experiencing a mental health emergency needing 24-hour care in a safe, supportive environment. Some programs may require traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services in a variety of socioeconomic neighborhoods.
