POSITION SUMMARY
The Care Manager for Central Intake is responsible for providing advocacy, coordination, and continuity of evidence-based care. The scope of work is within an Adult Inpatient Crisis Stabilization unit that serves individuals and families in need through a collaborative process of triage, assessment, planning, and ongoing care services, facilitated by effective communication. The Care Manager is a member of a multidisciplinary treatment team and reports to the supervisor of the unit or department. PRN staff are essential members scheduled on an 'as needed' basis to facilitate triage, assessment, and care for individuals.POSITION REQUIREMENTS A
minimum of a Bachelor's degree within a field of study in a Human Services-related discipline (i.e., Behavioral Health, Psychology, Psychiatry, Social Work, etc.) and at least 1 year of experience in the mental health field. (Child welfare programs require 2 years of experience with at-risk populations and a child welfare certification. ) Ability to read, analyze, and interpret clinical records, procedures, or governmental regulations. Ability to write reports and document clinical activities and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders. .POSITION RESPONSIBILITIES
Completion of thorough triage and psychosocial assessments and coordination of community resources based on patient needs as per program requirements. Complete all clinical and administrative documentation based on established timelines as per program requirements. Communicate effectively and maintain cooperative relationships with internal and external resources. Develop service plans, treatment plans, and/or aftercare plans with clear goals and objectives as per program requirements. Maintain efficient workflow management and organization as per program requirements. Maintain proficient quality of clinical documentation as per program requirements. Provides exceptional customer service and servant leadership in all communications and interactions with staff and the public. Adhere to patient privacy and confidentiality practices. Assists staff with support in all areas of functional responsibility in a courteous, supportive manner. Ability to sympathize with and comprehend the disorders faced by our patients and/or clients and their effects on behavior. Possess physical agility to safely and manually restrain patients (regardless of gender) during critical situations in a collaborative approach with team members. Other duties as assigned or needed as per program requirements.WORK ENVIRONMENT
High-paced, collaborative team performance with the emphasis on critical thinking and decision-making with guidance from the program supervisor/manager. The facility is a climate-controlled office environment with frequent exposure to mental health and behavioral health clients. Some programs may require traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services or onsite clinical programs in a variety of socioeconomic neighborhoods.PHYSICAL DEMANDS
Abilities include reaching, bending, talking, sitting, carrying, standing, grasping, fine hand coordination, hearing, reading, writing, and staying calm under stress. Must be able to reach with hands and arms, climb stairs, maintain balance, stoop, kneel, crouch, or crawl. Vision abilities required include close-up, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 50 pounds alone.
