Role:
Oversee the daily operations of Pearl Pointe, ensuring compliance with state and federal regulations Develop and implement policies and procedures to promote quality care and services Manage and coordinate the activities of the care team, including nurses, therapists, and other support staff Collaborate with healthcare professionals to develop and implement individualized care plans for each patient Ensure effective communication between patients, families, and healthcare providers Participate in quality improvement initiatives to continuously improve care and services
Responsibilities:
Leadership and Management:
Develop and manage the care team to ensure effective delivery of care services Foster a positive and respectful work environment Provide guidance and support to staff members
Care Coordination:
Develop and implement individualized care plans for each patient Coordinate with healthcare providers to ensure seamless transition of care Monitor and assess patient progress and outcomes
Quality Improvement:
Participate in quality improvement initiatives to identify and address areas for improvement Develop and implement quality improvement plans Monitor and evaluate the effectiveness of quality improvement initiatives
