Job Description

Schedule:
Monday - Friday 9am-5pm and as needed on weekends and evenings based on client and program needs.
Overview:
Boys & Girls Haven is seeking a full-time a Case Manager to support young adults in the Independent Living (IL) program. This role is responsible for securing and coordinating housing placements, providing ongoing case management, and supporting clients in maintaining stability and progress toward independent living.
Position Description:
The Case Manager provides case management services to clients in the IL program, including housing coordination, service planning, and support with daily living needs. This role works closely with clients and the treatment team to implement individualized plans and support progress toward independence.
Key Responsibilities:
Provide case management services to young adults in the IL program Coordinate housing placement, including securing units and preparing clients for independent living Assist clients with obtaining basic needs, including furnishings, food, and household items upon program entry Support clients in enrolling in school, obtaining employment, and pursuing vocational goals Connect clients to community resources and services necessary for stability Coordinate and monitor individualized treatment plans in collaboration with the treatment team Maintain accurate and timely documentation, including case files and progress updates Conduct regular meetings with clients to support progress and address needs Assist clients in understanding lease agreements, program expectations, and responsibilities
Qualifications:
Bachelor's degree in social services or related field required Minimum of two (2) years of experience working with older adolescents or young adults in a social services setting Experience providing case management services preferred Strong communication, organizational, and documentation skills Ability to work independently and manage multiple client needs Valid driver's license and insurable driving record Must be at least 21 years of age
