Memory Care Manager Qualifications:

This professional leads, guides, and directs the healthcare and general operations of the Memory Care neighborhood to include a quality Activity Program designed to meet the social, psychosocial and therapeutic needs of residents living with dementia.
Memory Care Program Coordinator Job Summary:
Directly supervise and train Resident Care Assistants working in the Memory Care neighborhood; recruiting, hiring, scheduling, managing call-outs. Provide administration of medications. Assess health, functional and psycho-social status of residents and prescribe approprite individualized service plans. Manage resident healthcare appointments and disseminate information to families and others on the care team. Participate in preadmission screening. Lead Resident care issue meetings. Maintain required documentation. Assist with the development of formal and informal staff educational in-service programs to ensure the highest level of quality care possible.
