Job Description

JOB DESCRIPTION DETAILS
Job Summary :
Responsible for following participating patients/beneficiaries from the hospital into their homes or a lower level of care facility using an evidence-based care coordination approach, provides services designed to streamline plans of care and interrupt patterns of frequent acute hospital or emergency department use and health status decline.
Education :
Bachelors degree. Current RN license. CCM preferred.
Registration/Certification/Licensure :
Register Nurse -
State of Maryland Experience :
5 years experience in case management in the hospital, home health or ambulatory care setting.
Other Requirements :
Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements. Effectively communicates with all patient populations and staff by implementing organization-wide communication techniques as a daily practice. Effectively advocates for quality patient safety and satisfaction as a daily practice. Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements. Demonstrated proficiency of computer skills necessary to effectively complete position requirements. Previous experience and demonstrated professionalism in effectively communicating with customers and in promoting a positive service atmosphere. Ability to work independently in a multi-task environment producing quality work. Specific technical skills may be required to fulfill department requirements. Up to six (6) months on-the-job training provided to gain proficiency.
