Job Description

The Case Manager is directly responsible to her/his supervisor who assigns cases, reviews clinical and administrative work and offers regularly scheduled supervision as well as additional consultation on an as-needed basis. The worker is expected to carry out routine assignments independently within the Agency's program policies and procedures and within her/his scope of responsibility.
Essential Functions:
Casework Practice:
Render direct casework to clients/families. Includes intake and evaluation process implementing the treatment plan; providing crisis intervention; and, analyzing and evaluating the course and results of the treatment process. Will include home and field visits. Perform all related activities that serve to benefit the clients including program-related case management. This may include contacting and working with other agencies/ facilities/individuals (referrals and ongoing collaboration); making full use of JSSA's resources; including arranging for homemaker, escort, etc.; providing information about other community resources (medical assistance, food stamps, transportation, nursing homes, etc.); and, serving as a client advocate as appropriate. Conduct or co-lead support, recreational and other groups, workshops and Family Life Education groups for PIC, CoA and other programs as assigned. Attend Coming of Age events, and outings and be available for participants who need social or case management services. Perform ACIS housing assessments in the community and upload all documents to the county health record in HMIS.
Consultation:
Act as a resource person to other JSSA professional staff on community resources, case management functions, etc. as assigned. Provide consultation/education to staff of collaborating community agencies/schools as assigned.
Administration:
Prepare and maintain current and accurate records, case recordings and statistics. Responsible for meeting outcomes required by grant funding for the programs assigned to this position. Submit all required forms and statistics on a timely basis. Set client fees and track the timeliness of client payments. Participate in department staffing, case conferences/consultation meetings as appropriate. Assist supervisor or director in coordination and planning of special initiatives/programs. Performs other duties are required.
Community Relations:
Establish close working contacts in the community, e.g. ACIS Housing providers, to facilitate referrals, collaborative implementation of treatment plans, follow-up, etc. Responsible for active community outreach and case finding strategies/efforts as assigned. Represent the department or Agency at community meetings, coalitions, etc.; speak to groups about the department's services, sometimes addressing specific problems, as requested by your supervisor. Participate in significant and continuing professional activities and community work, which enhance personal clinical skills and JSSA's image and impact on the community, this may include sitting for your licensure exam.
Minimum Education, Licensure, and Work Experience Required:
MSW or other advanced degree in related field.
