Social Work Aging Life Care Manager

Are you a licensed social worker seeking a fulfilling, relationship-driven role outside of a traditional clinical setting? Aging Well Eldercare, a well-established Aging Life Care Management practice serving the D.C. and the Maryland suburbs for over 37 years, is looking for a compassionate Social Worker to join our highly supportive and collaborative team.

Requirements:

Maryland Social Work license required; DC license preferred - support provided to obtain if needed. CCM or CMC certification a plus Ability to travel for in-person client visits in the D.C.
