Job Description

Job Overview We are seeking a dedicated and compassionate Care Manager to join our dynamic team, committed to enhancing the well-being and independence of individuals across diverse communities. In this vital role, you will coordinate comprehensive care plans, provide crisis intervention, and support clients through behavioral health challenges. Your leadership will empower clients to achieve their personal goals while navigating complex social and healthcare systems. Duties Develop individualized care plans that address clients' unique needs, including behavioral health, addiction recovery, child welfare, and social services. Conduct crisis intervention and crisis management to stabilize clients during urgent situations, ensuring safety and appropriate follow-up. Provide linkage to addiction counseling and behavioral health support to promote recovery and mental wellness. Coordinate with healthcare providers, social service agencies, and community organizations to facilitate access to resources such as Medicaid, Section 8 housing, and workforce development programs. Conduct motivational interviewing techniques to motivate clients toward positive behavior change and goal achievement. Maintain detailed documentation of client interactions, service plans, progress notes, and incident reports in compliance with regulatory standards. Skills Strong expertise in crisis intervention and crisis management strategies. Proven experience in working with clients in need of addiction counseling and behavioral health services. Knowledge of social work principles, child welfare systems, and Medicaid regulations. Excellent communication skills with the ability to motivate clients through motivational interviewing techniques. Familiarity with Section 8 housing programs and workforce development initiatives. Ability to handle sensitive situations involving child care needs or mental health crises with professionalism and compassion.
