Job Description
Montebello Health Group is expanding our Collaborative Care (CoCM) behavioral health program and seeking a compassionate, organized, and mission‑driven Behavioral Health Care Manager (BHCM) . This role blends clinical support , care coordination , and evidence‑based brief interventions within a team-based model that includes the Primary Care Provider (PCP) and a Psychiatric Consultant. The BHCM is the primary point of contact for enrolled patients and plays a central role in improving behavioral health outcomes through structured follow‑up, measurement‑based care, and warm collaboration with the care team.
