Job Summary We are seeking a dedicated and compassionate Care Coordinator - Chronic Care Manager to join our dynamic healthcare team. Responsibilities Develop and implement individualized care plans for patients with chronic illnesses, leveraging clinical expertise and patient needs. Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to inform treatment strategies. Manage patient intake processes, discharge planning, and post-acute care coordination to facilitate smooth transitions across care settings. Utilize electronic health records (EHR/EHR) systems to document clinical encounters, update medical records, and ensure compliance with HIPAA regulations. Monitor patient progress through regular follow-ups, adjusting care plans as needed based on diagnostic evaluations and clinical data. Engage in utilization management activities to optimize resource use while maintaining high-quality patient care standards. Experience Proven experience in critical care environments such as ICU, emergency room, or trauma centers is highly desirable. Strong background in hospital medicine, medical-surgical units, or post-acute care settings with familiarity in managing complex cases. Prior experience working within Level I or Level II trauma centers or infectious disease units is advantageous. Demonstrated expertise in case management, discharge planning, and outpatient or home health services. Knowledge of health regulation policies including HIPAA compliance and medical terminology is essential. Hands-on experience with EMR/EHR systems for documentation and patient record management. Excellent communication skills for client interaction and interdisciplinary collaboration are required to succeed in this role.
