Job Description

Experteer Overview In this role you drive integrated care for members with behavioral health needs, focusing on quality and cost-effective outcomes. You work with a multidisciplinary team to coordinate services across behavioral health, LTSS, and community resources, aiming to reduce emergency service use and support stable member progress. You engage high-acuity members through assessments, care planning, and ongoing outreach, employing motivational interviews to enable change. This position centers on ED diversion and coordinating rapid, person-centered care within a support-rich network.
Compensation / Benefits
  • Complete comprehensive behavioral health assessments within required timelines
  • Develop and implement person-centered care plans with the care team and support network
  • Conduct telephonic, in-home, or in-person visits as needed
  • Monitor and adjust care plans based on progress and barriers to outcomes
  • Maintain an ongoing member caseload with proactive outreach
  • Promote integration of behavioral health, LTSS, and community resources
  • Facilitate interdisciplinary care team meetings and collaboration
  • Use motivational interviewing and clinical guideposts to motivate change
  • Identify and address barriers to care and coordinate necessary support
  • Provide consultation and recommendations to peers as needed
  • Travel 25-40% to meet state/contract requirements Tasks
  • 2+ years healthcare experience, preferably in behavioral health
  • Licensed behavioral health clinician license (various) with active status in state of practice
  • Valid driver's license and reliable transportation for job-related travel
  • Experience with severe mental health concerns and substance use disorders
  • Knowledge of whole person care, chronic conditions, discharge planning
  • Data entry skills and experience with clinical platforms
  • Excellent verbal and written communication
  • Proficiency with Microsoft Office or applicable software Key requirements
