This opportunity allows you to join a dedicated team committed to making a positive impact in the lives of individuals and families affected by various types of dementia.
Position:
Part-time care manager position 16-24 hours per week in the office and remote work. All employees start at a part-time position and can gradually increase to full-time position as their client census increases; however full-time availably is not required. A full-time position is define as 32 hours a week.
Position Qualifications:
Hold a Bachelors or Masters degree in Social Work.
Required:
two years minimum of experience in community-based health care (e.g., home health, hospice, public health or senior services).
Preferred:
two years of experience in community-based health care (e.g. home health, hospice, public health or senior services). Must demonstrate clinical competency and show evidence of ongoing professional development. Mature, dependable, and responsible work ethic; stable and consistent work history. Professional verbal and written communication and people skills. Detail oriented with excellent organizational skills. Must be able to organize, prioritize, and manage multiple complex interpersonal and technical tasks, especially balancing on-site and remove work priorities. Must be able to use current version of technology proficiency and effectively such as internet, an iPad, laptop, cell phone, email system Microsoft Office Suite products. Before hire, must be able to pass a state and national criminal background check, credit history, and driver's license checks (i.e., free from any convictions related to this position). Must possess and maintain reliable transportation, basic Texas liability automobile insurance and current driver's license. Demonstrate skills in professional phone and email protocols
