Elumina Health and Altera Life Care are a locally owned and operated agency in Columbus, Ohio and we are adding to our team due to growth! Altera Life Care is a newly-licensed Hospice agency. To help drive our newest strategic initiative, we are seeking a skilled and compassionate individual to serve as the Director of Nursing & Administration for our hospice program. This is a new program, under development for our organization. This role will work very closely with the hospice administrator to develop operations for this startup. The Director of Nursing & Administration will provide leadership and oversee operations of our hospice services, ensuring the delivery of high-quality care to patients and their families during end-of-life care. Additionally, the Director of Nursing & Administration will provide full patient services during the startup period. This is a special opportunity for an experienced clinician to get in on the "ground floor" with a new agency providing a path to advancement in the organization. The ideal candidate will be patient care focused, organized, flexible, comfortable using a computer, and possess excellent communication & customer service skills.
Responsibilities:
Work closely with the hospice administrator to develop operations and procedures for the hospice vertical of the business. Monitor program performance and outcomes, identify areas for improvement, and implement strategies to enhance efficiency, effectiveness, and patient satisfaction. Receives case referrals. Reviews available patient information related to the case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with the Medical Director regarding any questions about an individual's eligibility for services. Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate. Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course with physicians. Completes initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of the nursing process. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plans of care. Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement. Participates actively in quality assessment performance improvement teams and activities. Conducts record reviews per QAPI program and communicates findings and recommendations to Clinical Director/Director of Patient Care Services and hospice personnel. Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new employees. Qualifications:
Registered nurse with current licensure to practice professional nursing in Ohio. Graduate of an accredited school of nursing with a BSN degree is preferred, or a minimum of three (3) years of experience as a professional nurse within the last five (5) years. Previous experience in hospice setting and with two (2) years management or supervisory experience. Proven ability to work within an interdisciplinary setting. Complies with accepted professional standards and practice. Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying. Understands principles of pain/symptom management. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Complies and maintains current CPR certification. Demonstrates excellent observation, verbal and written communication skills.
Functional Abilities:
Must be able to read 12 point type or larger. Must be able to communicate verbally and in writing in a manner understood by most persons. Must be able to stoop, bend and be able to lift and transfer clients. Must be able to travel to clients' residences.
Special requirements:
Must have a reliable car, car insurance and a valid driver's license. Must be able to successfully pass a background check.
