ElderCare Connections is seeking a dependable, organized, and experienced Social Work Care Manager to support older adults and their families throughout the Fredericksburg region. We are a small, growing geriatric care management company looking for someone who has a strong commitment to helping older adults and is interested in growing with our team for the long term. The ideal candidate will have excellent critical-thinking and problem-solving skills. They must be comfortable entering complex situations with limited information, assessing the client's needs, identifying priorities and risks, and using their professional knowledge and judgment to develop practical next steps. This role requires independence, sound judgment, strong communication, and consistent follow-through.
Responsibilities Responsibilities may include:
Visiting clients in homes, hospitals, rehabilitation centers, assisted living communities, memory care settings, and skilled nursing facilities. Assessing psychosocial needs, family dynamics, safety concerns, support systems, and gaps in care. Supporting clients and families through dementia, chronic illness, caregiver stress, placement decisions, and care transitions. Attending medical appointments, care plan meetings, facility meetings, and discharge planning meetings. Coordinating referrals, community resources, home care, home health, hospice, transportation, and other services. Communicating with clients, families, providers, facilities, and community partners. Completing timely and professional documentation. Tracking outstanding needs and ensuring follow-up is completed. Recognizing concerns that require leadership involvement or escalation. Building positive relationships with referral sources and community partners.
Qualifications The ideal candidate will:
Hold a Bachelor of Social Work degree or higher . Social work licensure is welcome but not required. Have experience working with older adults, families, healthcare systems, or community-based services. Demonstrate strong critical-thinking, assessment, and problem-solving skills. Be resourceful, dependable, organized, and calm under pressure. Work independently while communicating consistently with the ECC team. Manage multiple client needs and competing priorities. Communicate clearly and professionally. Complete documentation and follow-up promptly. Have a strong desire to support older adults and their families. Be interested in building a long-term role with a small, growing company. Experience in geriatric care management, case management, hospital social work, discharge planning, hospice, home health, assisted living, memory care, skilled nursing, or aging services is strongly preferred. Schedule and Travel Applicants must have consistent availability at least three full days per week . Assignments and billable hours will vary depending on client needs and caseload. Reliable transportation and local travel are required throughout Fredericksburg, Stafford, Spotsylvania, King George, Bowling Green, Caroline County, Orange County, and surrounding areas. Compensation This is a 1099 independent contractor position paying $48-$55 per approved billable hour , depending on experience, qualifications, and fit. Billable work may include client visits, appointments, documentation, family and provider communication, referrals, care coordination, and other approved client-related services.
