Bachelor's degree in counseling, psychology, social work, behavioral sciences, or related field. Salary scale based on experience. Job Summary We are hiring a full-time Care Manager to provide person-centered support to adults and children. This role helps individuals access medical, behavioral health, social, and community resources following the TN Health Link model. Services are delivered in the office and community. Key Responsibilities Develop and monitor individualized care plans. Coordinate medical and behavioral health appointments. Connect individuals to community resources (housing, food, employment, legal, etc.). Provide in-person support for treatment and medication adherence. Assist with hospital discharge planning and crisis support. Educate patients and families on independent living skills. Maintain accurate documentation and reports. Work collaboratively with team members, families, and providers. Qualifications Bachelor's degree behavioral sciences required (Counseling, Psychology, Social Work, or another related health field). Strong communication, organizational, and interpersonal skills. Ability to work both independently and as part of a multidisciplinary team. Must demonstrate empathy, professionalism, and reliability.
