Job Summary:
The Memory Care Coordinator provides direct personal care and supervision of the memory care residents at the Community. Promotes resident well-being and satisfaction through support with activities of daily living and coordinating and executing engaging activities. Communicates with other departments to ensure residents needs are met. Assumes the responsibility of supervision and delivery of resident care and engagement in compliance with Silverpoint expectations, policy and procedure and State regulations.

Requirements:
Strong assisted living resident care background MUST have a big heart and be willing to work in an Memory Care Setting with individuals w/ Alzheimer's & dementia. At least two years' experience in working with the public in face-to-face interaction, preferably with senior living residents. Sensitive and responsive to prospects' and resident's needs. Tactful and courteous when dealing with residents and team members. Experience and strong computer skills with
Microsoft Office:
Word, Excel, Outlook, PowerPoint and database management for statistical reporting. Good judgment and decision-making skills. Ability to maintain confidentiality. Ability to deal with multiple priorities/demands. Professional demeanor and appearance. Effective oral and written communication in English. Excellent organizational skills. Able to work a flexible schedule to support occasional weekend move-ins and periodic after hours' events as needed. Ability to Lift 50lbs.
Reliable Transportation Essential Duties:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/ or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Meets Silverpoint's standards established for the resident care department. Assists Wellness Director to ensures resident care staffing is within regulatory and company requirements Assists Activities Director in ensuring engaging programming and activities for residents are scheduled and executed in the Memory Care department. Performs daily filing of all documents relevant to the resident record in chronological order. Performs a quality assurance review of the documents prior to filing to ensure items are dated, verified, and signed by the physician if appropriate. May conduct weekly cart audits of each medication cart following the cart audit process and submits the audit sheet to Health & Wellness Director and Executive Director. Performs a daily review of medication administration process ensuring medications are passed timely and accurately. Intervenes as needed to provide support/ training during the med pass. Follows the medication order process and maintains adequate records of order transcription and verification. Notifies the Health & Wellness Director or Executive Director of changes in resident's condition, and any unusual or abnormal observations. Schedules appointments, and arranges for transportation of residents for diagnostic, therapeutic, dental or medical services. Ensures compliance with the physician visit log process/procedure. Assists the Health & Wellness Director to ensure compliance with all required logs, policies and procedures relevant to medication administration. Ensures care staff understands their role as it relates to dietary services including how to serve, the requirement to pour water at every meal, and how to ensure the right resident receives the right diet prepared the right way according to the posted diet board. Assists to serve residents during mealtime in the Memory Care dining room. Assists with transporting residents to/from appointments utilizing the community van, as necessary. Performs building rounds at the beginning and end of each shift, evaluating the care provided and ensuring resident needs are met. May be required to work overtime as necessary. Participates in weekend call in rotation with other managers. Participates in Manager on Duty Program. Assists in the evacuation of residents as necessary. Maintains the confidentiality of all resident information and honors the residents' personal property rights. Performs any other duties that may be assigned by the Health & Wellness Director or Executive Director. Performs other duties as assigned and which relate to the success of SilverPoint or the Community.
