Position Overview Teays Valley Place is seeking an experienced and compassionate Memory Care Director ( Certified Dementia Practitioner CDP ) to lead our specialized memory care neighborhood. In this essential leadership role, you will oversee the daily operations of the memory care program, ensure a seamless transition for new residents, develop engaging dementia-specific programming, and promote the highest quality of life for those we serve. This position is ideal for a Certified Dementia Practitioner (CDP) who is passionate about person-centered dementia care, possesses strong leadership skills, and is committed to creating a safe, engaging, and supportive environment for residents and their families. Key ResponsibilitiesResident Admissions & Care Planning Coordinate all aspects of the admission and transition process for new memory care residents. Complete comprehensive assessments to identify each resident's cognitive, physical, emotional, and social needs. Collaborate with families, physicians, and interdisciplinary team members to develop individualized care plans. Monitor resident progress and update care plans as needed. Programming & Resident Engagement Develop and implement meaningful, therapeutic programming designed specifically for individuals living with Alzheimer's disease and other dementias. Lead activities that incorporate reminiscence therapy, sensory stimulation, cognitive engagement, music therapy, and social interaction. Adapt programming to meet each resident's individual abilities, interests, and level of cognitive function. Encourage resident participation while promoting dignity, purpose, and independence. Clinical & Resident Support Provide hands-on assistance with Activities of Daily Living (ADLs) while preserving each resident's dignity and independence. Administer medications in accordance with state regulations and company policies. Observe and report changes in resident condition to the appropriate clinical staff. Maintain accurate clinical documentation and medication records. Leadership & Team Development Lead, coach, and mentor memory care staff while promoting a positive, resident-centered culture. Educate team members on dementia care best practices, behavior management techniques, and person-centered care approaches. Foster an environment built on compassion, respect, safety, and teamwork. Ensure compliance with all federal, state, and company regulations. Qualifications Certified Dementia Practitioner (CDP) Certification (Required) Minimum of 2 years of experience working in memory care or dementia-specific programming. Previous leadership experience in assisted living, memory care, skilled nursing, or another long-term care setting preferred. Strong understanding of Alzheimer's disease, related dementias, and evidence-based behavior management strategies. Experience assisting residents with Activities of Daily Living (ADLs). Knowledge of medication administration and clinical documentation. Excellent communication, leadership, and organizational skills.
