This is a full-time, salaried leadership role responsible for interdisciplinary team oversight, clinical quality, regulatory compliance, and staff development across a home-based hospice program. The position reports to the Program Director and works closely with the hospice physician, community liaisons, nursing staff, aides, social workers, and chaplains. Schedule Monday through Friday, 8:00 a.m. to 4:30 p.m., with participation in an after-hours on-call leadership rotation. Field presence within the Marin and Sonoma County service area is required as clinical and team support needs dictate. Compensation $155,000 to $165,000 base salary, depending on experience, plus eligibility for a quarterly bonus program of up to 10% of base salary. What You'll Do You will provide clinical leadership to an interdisciplinary hospice team delivering patient-centered end-of-life care across Marin and Sonoma Counties. You will coordinate and oversee patient services in alignment with hospice program policies and applicable federal and state regulatory requirements, and will lead or participate in interdisciplinary team meetings to review patient and family care plans, address clinical challenges, and ensure continuity of care. You will conduct chart audits and maintain oversight of medical record quality, documentation timeliness, and compliance standards. You will lead survey readiness efforts and support the organization through regulatory review processes. You will participate in clinical staff orientation, in-service education, and ongoing training, and will evaluate, coach, counsel, and develop clinical team members to improve individual performance and overall service delivery. You will collaborate with operational, physician, and community liaison partners to support quality care, census management, and referral responsiveness. You will manage terminal care situations, crisis intervention, pain and symptom control, and family support at the clinical leadership level, providing field support to nursing staff as needed. You will also participate in the after-hours on-call leadership rotation. Required Qualifications Current, unencumbered Registered Nurse license in the state of California Minimum 2 years of recent direct patient care experience in hospice, home health, or community health Minimum 1 year of clinical supervisory experience Experience with regulatory compliance, survey processes, chart audits, and clinical quality oversight Proficiency with EMR systems used in hospice or home health settings Current CPR certification Valid California driver's license, current vehicle insurance, and reliable personal transportation Preferred Qualifications Bachelor of Science in Nursing (BSN) Hospice clinical experience, including familiarity with Medicare hospice conditions of participation Certified Hospice and Palliative Nurse (CHPN) credential, or willingness to pursue certification Experience leading interdisciplinary teams in a home-based or community care setting Strong home health clinical leadership experience may be considered in lieu of direct hospice leadership experience Familiarity with census management, referral coordination, and liaison collaboration in a hospice or home health market Compensation and Benefits Base salary of $155,000 to $165,000, depending on experience Quarterly bonus program, up to 10% of base salary Medical, dental, and vision insurance 401(k) with company match 15 days paid time off plus 10 paid holidays Tuition reimbursement and certification assistance Cell phone, mileage, and gym membership reimbursement Wellness and employee discount programs Structured clinical onboarding program To apply, submit your resume along with a brief summary of your hospice or home health clinical leadership experience and your familiarity with regulatory compliance and survey processes.
