This position focuses on continuity of care, chronic disease management, utilization review, discharge planning, and coordination of services among medical, behavioral health, custody, and community-based providers. The RN Care Manager works with an interdisciplinary healthcare team to ensure patients receive timely, appropriate, and cost-effective care while maintaining compliance with institutional policies, professional nursing standards, and correctional security procedures. The ideal candidate demonstrates sound clinical judgment, strong organizational skills, cultural sensitivity, and the ability to work effectively in a secure and highly structured environment. Essential Duties and Responsibilities Conduct comprehensive nursing assessments to identify patients with complex medical, behavioral health, social, or transitional care needs. Develop, implement, and regularly update individualized care plans in collaboration with physicians, advanced practice providers, nursing staff, behavioral health professionals, custody personnel, and other members of the healthcare team. Coordinate medical appointments, specialty consultations, diagnostic testing, hospitalizations, follow-up care, and other healthcare services. Monitor patients with chronic and high-risk conditions, including diabetes, hypertension, cardiovascular disease, respiratory disorders, infectious diseases, substance-use disorders, and other complex healthcare needs. Review patient progress, treatment adherence, laboratory results, medications, and clinical outcomes to identify barriers or changes in condition. Facilitate timely communication between institutional healthcare providers, outside hospitals, specialty providers, pharmacies, and community agencies. Participate in utilization management activities to ensure services are medically necessary, clinically appropriate, and delivered at the proper level of care. Coordinate discharge and reentry planning for patients preparing for release or transfer to another facility. Arrange continuity of medications, medical equipment, follow-up appointments, health education, and referrals to community healthcare resources when applicable. Identify social determinants of health that may affect treatment outcomes, including housing instability, transportation, access to medication, health literacy, and family or community support. Provide patient education regarding diagnoses, medications, treatment plans, preventive care, chronic disease management, and healthy lifestyle practices. Advocate for patients while maintaining professional boundaries and complying with correctional safety and security requirements. Respond appropriately to changes in patient condition and communicate urgent concerns to the appropriate medical provider. Participate in interdisciplinary case conferences, chronic care clinics, treatment team meetings, discharge planning meetings, and quality-improvement activities. Maintain accurate, timely, and complete documentation in the electronic health record. Protect patient confidentiality and handle protected health information in accordance with applicable privacy requirements and institutional policies. Assist with tracking referrals, pending appointments, hospital returns, missed services, and follow-up requirements. Support infection prevention, medication safety, patient safety, and risk-management initiatives. Follow all facility procedures regarding security, controlled movement, emergency response, tool and equipment control, and professional conduct. Report suspected abuse, neglect, exploitation, or significant safety concerns according to applicable reporting requirements. Perform additional nursing and care-management duties as assigned. Minimum Qualifications Current and unrestricted Registered Nurse license in the state of employment. Graduate of an accredited professional nursing program. Current Basic Life Support certification from an approved provider. Minimum of three years of professional nursing experience. At least one year of experience in case management, care coordination, utilization review, discharge planning, chronic disease management, or a related clinical role. Ability to successfully complete all correctional facility background, security, credentialing, and health-screening requirements. Ability to communicate effectively with patients, healthcare professionals, custody staff, outside providers, and community organizations. Proficiency with electronic health records and standard computer applications.
