5 × 8-hour shifts per week (40 hours/week) Job Description We are seeking an experienced Registered Nurse (RN)
  • Care Manager/Case Manager for a 13-week travel assignment in Saint Helena, California.
The RN Care Manager will coordinate and manage the continuum of care for patients from pre-admission through post-discharge. This role works closely with patients, families, physicians, nurses, multidisciplinary teams, and healthcare partners to ensure safe, efficient, and high-quality patient outcomes. The position supports adult psychiatric and behavioral health units , including a Medical Behavioral Health unit and a traditional Adult Psychiatry unit. Candidates with a background in behavioral health are strongly preferred, along with solid case management experience. Key Responsibilities Coordinate care for patients throughout the hospitalization and discharge process. Collaborate with physicians, nurses, patients, families, and multidisciplinary teams. Develop, implement, evaluate, and revise individualized care plans. Serve as a liaison between patients, families, healthcare providers, and insurance/healthcare benefit representatives. Coordinate appropriate resources and services to support patient needs after discharge. Monitor patient progress and help ensure efficient movement through the continuum of care. Provide case management consultation and support to the healthcare team. Assist in optimizing patient outcomes while maintaining quality and cost-effective care. Maintain accurate and timely documentation in the electronic medical record. Requirements Active Registered Nurse (RN) license in the state of California. Minimum 2 years of nursing experience . Previous Case Management or Care Management experience preferred. Behavioral Health/Psychiatric nursing experience strongly preferred. Previous experience working with adult psychiatric or behavioral health populations is a plus. Epic experience required. Current BLS certification required. TEAMS or CPI certification required. BSN preferred. Case Management certification preferred. Schedule & Additional Details Day shift: 8:00 AM
