ASAP Responsibilities Perform comprehensive patient assessments and develop individualized care plans. Provide skilled nursing care in patients' homes. Coordinate care with physicians and other healthcare professionals. Educate patients and families on medications, treatments, and disease management. Monitor patient progress and update care plans as needed. Maintain accurate and timely clinical documentation. Ensure compliance with home health regulations and agency policies.
