Job Description

Job Overview The Madrone Care Network (MCN) RN Census Manager is a registered palliative care nurse tasked with the evaluation and implementation of health care plans for individual patients. Responsibilities consist of assessment, planning, implementing, coordinating, monitoring, and evaluating the options and services required to meet the patient's health and service needs. The RN Census Manager is part of an interdisciplinary palliative team and is responsible for overseeing the physical, psychosocial, and spiritual care needs of each of the patients on their census as well as deploying dedicated census team members to meet routine and urgent patient needs. This position will serve a Lake County census of patients and requires both tele-health and in person visits.
ESSENTIAL JOB FUNCTIONS
Initial in-person on service evaluation Ongoing patient care relationship and care interface including the Zoom enabling process Initiation, management, and evolution of the patient's individualized care plan Patient and family/caregiver communication Adjusting the frequency/type of contacts based on patient need Internal and external resource utilization and coordination AD and POLST preparation, completion, and periodic patient review Documentation for patient recertification or recommending disenrollment to the
IDT. SECONDARY JOB FUNCTIONS
Models MCN's Mission, Core Values, and The Madrone Way Working collaboratively with other members of the Interdisciplinary Team (IDT) who are in service to them and their patients Preparing the IDT Template and leading the weekly meeting, ensuring that the notes from the meeting appropriately capture the discussion and action steps Determining the appropriate Continuity Nurse and Team caring for each patient on census with attention to team member/patient continuity Ensuring delegation of Census Manager responsibilities to other clinical staff when on vacation/leave Assuring that required visits and contractual obligations for their census population are met Ensures effective use of resources Completing care coordination and chart notes within standards set by MCN Maintaining MCN confidentiality standards Taking responsibility for and demonstrates safe work practices Evaluating annually the performance of Dedicated Census Team members Performing other tasks as assigned
ESSENTIAL EQUIPMENT, TOOLS OR MACHINERY
The following MCN equipment will be used to effectively perform the described job functions(MCN provides equipment minus employee personal vehicle): Laptop computer or I-pad Cell phone Stethoscope, blood pressure cuff, thermometer and portable stand-on scale Teleconferencing software Personal vehicle
WORKING CONDITIONS
Work may be conducted from MCN's office, a remote and electronically secure location that has stable WiFi and internet connectivity, or a patient's residence. Requires frequent sitting, walking, and standing. May have some exposure to biological hazards and infectious diseases.
QUALIFICATIONS
Current California Driver's License, auto insurance and vehicle registration Current and ongoing California nursing license Basic computer skills with capacity to access patient records and chart appropriately. Able to teleconference with patients and their families Capable of hearing and communicating verbally with patients, caregivers, other members of the team, and community partners Ability to respect all patients irrespective of their circumstance, ethnicity, race, sexual orientation, religious belief, or political affiliation Ability to perform the essential functions of the job
