Job Description

Cobalt Healthcare and Rehabilitation Center is a facility whose mission is to care for our medically fragile patients and make a real difference for our at-risk elderly community. Cobalt Health Care and Rehabilitation Center is looking to hire a compassionate full-time LPN Charge Nurse to provide skilled nursing care to our patients in the home-like nursing setting.
Qualifications:
Must have good standing license (LPN) in the state of Connecticut Must have at least 6 months of hands-on nursing experience Current CPR certification Current IV Certification Weekend availability Responsibilities of the position: One to one patient care in a nursing home environment Execution of physicians prescribed plan of care and compliant documentation of care provided in system of record Administration of prescribed medication, treatments and therapies Patient assessments, coordination of care (RN) Oversee the CNA staff in providing ADL assistance and care of the residents. Work in collaboration with the RN supervisors, Director of Nursing and Administrator to provide quality care of the residents in our facility. Come join a fabulous team!
