Job Description

The Clinical Operations Lead is a field-based clinical lead responsible for driving performance across the post-acute continuum, with a focus on ACO and skilled nursing facilities. This role partners closely with SNF leadership, hospital teams, providers, and internal care coordination teams to improve transitions of care, reduce avoidable hospital utilization, and enhance patient outcomes. This individual will serve as a key operator in executing Spectrum's value-based care model-ensuring patients are proactively managed, workflows are standardized, and data-driven interventions lead to measurable improvements in readmissions, length of stay, and overall quality outcomes. Candidates must be located in the Orlando, FL area. Salary range is $85k-95k, based on experience and skillset. Core Responsibilities 1. Value-Based and ACO Performance Ownership
  • Co-own performance across key metrics: readmissions, LOS, avoidable utilization
  • Lead root cause analysis and action planning 2. Clinical Operations & Workflow Optimization
  • Partner with SNF leadership to identify areas of collaboration and quality improvement
  • Implement and collaborate on best-practice workflows (admissions, change-in-condition, discharge) 3. Transitions of Care & High-Risk Patient Management
  • Support SNF partnership meetings and care planning in partnership with providers
  • Ensure effective care transitions hospital → SNF → home with supportive resources 4. Right Care, Right Place, Right Time
  • Reinforce pre-transfer protocols and care transitions strategies
  • Support appropriate level-of-care decisions 5. Care Team Collaboration
  • Facilitate interdisciplinary collaboration across care teams supporting patient plans of care 6. Data-Driven Performance Improvement
  • Use dashboards to identify trends
  • Translate insights into action plans
Qualifications Required:
  • RN, NP, or clinical licensure preferred
  • 5+ years in post-acute care, care coordination, or value-based care
  • Strong understanding of transitions of care
Preferred:
  • Experience with ACO/MSO models
  • Familiarity with SNF operations and EMRs (Epic, Point Click Care) Key Competencies
