Job Description

MDS Coordinator (RN or LPN) Join a highly respected senior care organization where you'll play a key leadership role in driving resident outcomes, care planning, and reimbursement accuracy. In this position, you'll lead the MDS and PDPM process, coordinate interdisciplinary care plans, ensure regulatory compliance, and collaborate closely with residents, families, and clinical teams to deliver exceptional care.
What We're Looking For:
Active Florida RN or LPN license Experience with MDS, PDPM, and care planning is a MUST in LTC. Strong clinical assessment and documentation skills Excellent communication and leadership abilities Passion for providing high-quality care to seniors
