• Opportunity to grow with our company What You'll Do As a Care Manager, you'll serve as a trusted advocate and resource for clients and families, helping them navigate aging, healthcare, and changing care needs.

Responsibilities include:

• Conducting assessments and developing individualized care plans
• Providing home visits, wellness checks, and ongoing monitoring
• Coordinating medical care, appointments, home care, and other services
• Communicating with physicians, healthcare providers, and family members
• Advocating for clients during medical appointments and care transitions
• Providing dementia and memory-care support
• Identifying changes in health, safety, or functional needs
• Helping families navigate hospital, rehabilitation, assisted living, and memory-care transitions
• Providing education, guidance, and caregiver support Who We're Looking For RN preferred, with experience in geriatrics, healthcare, care management, home health, dementia care, or a related field.
