As a Clinical Supervisor, you will play a vital role in ensuring the highest quality of patient care and overseeing the daily operations of our home health team. Supervise and coordinate the activities of a team of home health nurses , therapists, and other healthcare professionals to ensure effective and efficient patient care delivery Conduct patient assessments, develop and implement care plans, and provide guidance and support to staff members Investigate and resolve patient complaints , ensure compliance with regulatory requirements, and maintain accurate records and reports Collaborate with physicians, social workers, and other healthcare professionals to ensure seamless communication and coordination of patient care Participate in on-call rotations, respond to emergencies, and provide guidance to staff members after hours Develop and implement quality improvement initiatives , monitor patient outcomes, and identify areas for improvement Recruit, train, and mentor staff members to promote professional growth and developmentRequirements:
Current Illinois RN license and BSN degree in Nursing or related field At least 3 years of experience as a registered nurse within the last 5 years, and with a minimum of 2 years in home health care Strong leadership and communication skills , with the ability to work effectively in a fast-paced environment Excellent clinical knowledge and critical thinking skills , with the ability to analyze complex patient situations and develop effective care plans Ability to lift up to 25 pounds, travel to patient homes, and work in a dynamic environmentBenefits:
