Job Description

Valley Professionals Community Health Center is looking to hire a RN Care Manager for our Crawfordsville location. Valley Professionals is a Federally Qualified Health Center located in several locations throughout west-central Indiana. Our mission is to provide comprehensive integrated healthcare for all individuals and families while promoting health education opportunities for the community, students, and health care professionals. Valley Professionals offers excellent benefits, including: Base salary of $61,800-$83,470 Health, dental, and vision insurance with Health and Flexible Savings Account options Employer paid life insurance, short-term and long-term disability Generous paid time off 8 paid holidays (separate from PTO) 403b Retirement plan with 6% match after 1 year of employment Tuition reimbursement up to $4,000 per year Qualified for Public Student Loan Forgiveness Job duties: This position will work directly with patients providing health education and working with the patients to improve health outcomes. The RN Care Manager will manage high risk patient care, including the management of patients with multiple co-morbidities, and will coordinate the continuity of patient care with external healthcare organizations.
Requirements:
