Job Description
Valley Professionals Community Health Center is looking to hire a RN Care Manager for our Crawfordsville location. Valley Professionals is a Federally Qualified Health Center located in several locations throughout west-central Indiana. Our mission is to provide comprehensive integrated healthcare for all individuals and families while promoting health education opportunities for the community, students, and health care professionals. The RN Care Manager will manage high risk patient care, including the management of patients with multiple co-morbidities, and will coordinate the continuity of patient care with external healthcare organizations.
