DUTIES AND RESPONSIBILITIES
Provide patient care management to high-risk patients. Provide needed nursing services to patients including medication and vaccine administration as directed by providers, immunizations, patient education, referrals to other providers and community health resources. Monitor patient flow and assist clinical support staff in preparation of examination rooms, preparation of patients for exam, and taking vital signs of patients. Serves as clinical resource for nursing staff. Triage medical calls and same day patients. Supervise nursing functions and create daily/weekly team nursing schedule. Represent medical support staff on HPC quality improvement initiatives. Maintain a role in hiring and training nursing staff. Attend events on site or offsite to work the events, set up, and clean up. Perform other related duties as may be assigned by the Clinical Director and/or Chief Health Officer. Perform all duties and services in full compliance with HPC policies and procedures. Regular attendance and punctuality are essential requirements of this job.
EOE QUALIFICATIONS
Ability to pass requirements per HPC policy (i.e., verification of credentials and privileges granted by way of executing this Position Description). Possession of a valid unencumbered Kansas RN license. Ability to relate well with patients and other health care personnel. Social and cultural sensitivity appropriate to ethnically and economically diverse patient-and employee-base. Willingness to educate patients and the community in health maintenance and the role of the health care team. Ability to work under time-based priorities. Commitment to expanding access to care for the underserved. Excellent time management skills and ability to motivate their teams.
EDUCATION AND EXPERIENCE
High School Diploma or GED is required. Graduation from a certified RN program. Experience within a community health center or similar environment preferred. Current unencumbered RN license in the State of Kansas without any restrictions. At least two years of nursing experience in an ambulatory clinic is required. Previous supervisory experience preferred.
TYPICAL PHYSICAL REQUIREMENTS
Prolonged sitting or standing may be required. Ability to lift 25 pounds; occasionally up to 60 pounds. Those physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with office work will be performed on a repetitive basis. The ability to distinguish letters and symbols as well as the ability to utilize telephones, computer equipment and printers is required. Possible exposure to communicable disease, body fluids, medicine preparation, and other conditions common to a clinical environment may be encountered. Under the direct supervision of the Clinical Director, the Nurse Clinic Manager performs patient care management, nursing staff management, procurement of clinic supplies, and management of equipment servicing. Working collaboratively with leadership, this position supports the advancement of the company's vision, mission, and values with day-to-day process support and quality improvement support. Health Partnership Clinic (HPC) employs a team-based approach to care delivery for all patients. The Nurse Clinic Manager will work with the medical support team to effectively participate in huddles at the start of each clinic day, use evidence-based approaches to patient care such as teaching chronic disease self-management, emphasize seamless care coordination for all patients, strive to understand the vulnerable populations served, and adjust services to fit patient needs. Upon hire, all staff receive orientation regarding benefits of the HPC team-based approach to overall population health.
