Home Health Licensed Practical Nurse (LPN) Clinical Coordinator In-Office | Braintree, MA | Full Time (No weekends or On-Call) At Elara Caring, we believe the best care happens where people feel most comfortable—at home. We are looking for a compassionate and detail-oriented Licensed Practical Nurse (LPN) Clinical Coordinator to join our Braintree office team. In this role, you'll play a key part in supporting home health clinicians and patients by helping coordinate care, manage clinical documentation, and ensure patients receive timely, high-quality services. Support quality care delivery by promoting Elara Caring's mission and clinical standards Review and process coordination notes and clinical workflow tasks in HomeCare HomeBase (HCHB) Obtain and enter physician orders (verbal, written, and faxed) for clinical review and approval Maintain and update patient tracking tools, including at-risk registries Assist with scheduling, follow-ups, and communication between patients, caregivers, and care teams Help obtain prior authorizations and support administrative clinical needs Conduct chart audits to ensure compliance with company policy and regulatory standards Verify required patient notices and documentation (including Medicare-related forms) Maintain strict confidentiality in accordance with HIPAA guidelines Provide occasional support with field visits or direct patient care, as needed Perform additional duties to support the clinical team and patient care operations•What We're Looking For Graduate of an accredited Licensed Practical Nursing program Active, unrestricted LPN license in Massachusetts Minimum 1 year of nursing experience in a clinical setting Strong attention to detail and organizational skills Comfortable working in an office-based clinical coordination role Ability to travel within the branch coverage area if needed Valid driver's license, insurance, and reliable transportation (if travel is required) Ability to sit, stand, bend, lift, and move intermittently; able to lift 50-100 lbs as needed•Join Our Team If you're an LPN who enjoys being part of a fast-paced, supportive clinical team and wants to make an impact beyond the bedside, this role offers a meaningful next step in your career.
