This role as Hospice Clinical Manager will lead our hospice care team with excellence and compassion. The ideal candidate will possess extensive clinical expertise, leadership abilities, and a passion for providing exceptional end-of-life care. As the Hospice Clinical Manager, you will oversee patient care delivery, coordinate interdisciplinary teams, ensure compliance with healthcare regulations, and foster a supportive environment for both staff and patients. Lead and supervise hospice nursing staff, ensuring high-quality patient care aligned with organizational standards and regulations. Oversee clinical operations, including patient assessments, care planning, and symptom management to optimize comfort and dignity. Coordinate interdisciplinary team efforts to develop comprehensive care plans tailored to individual patient needs. Ensure compliance with HIPAA, state healthcare regulations, and accreditation standards such as NCQA standards. Manage documentation processes utilizing EMR systems Conduct staff training on clinical protocols, infection control practices, IV infusion procedures, and medication administration. Facilitate discharge planning, family education, and coordination with community resources to support ongoing patient needs. Promote a positive team environment by fostering communication skills, managerial support, and professional development opportunities. Extensive clinical experience in hospice or palliative care settings with knowledge of symptom management and end-of-life protocols. Strong leadership skills with proven experience managing nursing teams in complex healthcare environments. Proficiency in EMR/EHR systems such as Epic or Cerner; familiarity with medical documentation standards. Expertise in managing care within diverse settings such as assisted living facilities, nursing homes, or home health environments. Familiarity with medical terminology related to geriatrics, behavioral health, dementia care, and pediatric populations including neonatal ventilator management. Ability to perform comprehensive patient assessments involving vital signs, physical examinations, and physiological evaluations. Skilled in medication administration including injections, IV insertion, catheter care, and sterile techniques. Experience working within state healthcare regulations governing long-term care facilities and hospice services. Strong communication skills for effective team supervision, family interactions, and interdisciplinary collaboration.
