Charge Nurse

Summary/ObjectiveIn keeping with our organization's goal of improving the lives of the Residents we serve, theed Nurse (R.N.)/Licensed Practical Nurse (L.P.N.) plays a critical role in providing superiorcustomer service and nursing care to all Residents and residents. The

providessupervision of staff and will safeguard the health, safety, and welfare of all Residents/residentsunder their care by following applicable laws, regulations, and established nursing policies andprocedures.

Provides quality nursing care to Residents/residents in an environment that promotes theirrights, dignity, and freedom of choice.2. Provides supervision to G.N.A's and all subordinate staff which includes checking their workto ascertain that assignments have been completed.3. Assign all G.N.A's and other subordinate staff to Residents. Assign break times and allregular and special assignments.4. Administer all parenteral, intramuscular, and sub-cutaneous injections.5. Administer medications within the scope of practice of the

R.N./L.P.N.

Licensure. Maintain acurrent report of narcotics received and used.6. Review daily the documentation of the dispensing of the controlled substances andnarcotics.7. Place pharmacy orders, for and administer all newly prescribed medications and document.8. Carry out direct contemporaneous charting in your shift. Chart detailed monthlyevaluations of each Resident that reflect his/her condition and progress.9. Responsible for all nursing care of assigned Residents while on duty. Must notifyappropriate persons if t is any significant change in a Resident's condition or anytransfer to a hospital.10. Ensure that Resident care plans are being followed and asses each Resident's status inaccord with their care plan.11. Round with physicians when needed.12. RN's to assess residents for change in clinical condition and prn.13. LPN's to monitor residents for change in clinical condition and prn.14. Administer or supervise all treatments prescribed by physicians including but not limited topressure ulcer care, Foley catheter care.15. Supervise serving of prescribed diets and fluids intake and immediately report any issues tothe physician and

Must be knowledgeable of individual resident focused care plans and support the careplanning process by reporting specific information and observations of the Resident'sneeds, preferences and report any behavioral changes.17. Provide wound care when needed.18. Completes medical records documenting care provided and other information inaccordance with nursing policies while maintaining strict confidentiality.19. Assure that established infection control and standard precaution practices are maintainedwhen providing care. Follow established safety precautions when preforming tasks andusing equipment and supplies.20. Reports all hazardous conditions, damaged equipment and supply issues to appropriatepersons.21. Maintains the comfort, privacy and dignity of Residents/residents and interacts with themin a manner that displays warmth, respect and promotes a caring environment.22. Answer and respond to call lights promptly and courteously.23. Assist with new admissions, re-admissions and assist with the transfer of Residents todifferent rooms within the facility.24. Communicates and interacts effectively and tactfully with Residents/residents, visitors,families, peers and supervisors.25. Attend and participate in departmental meetings and in-services as directed.26. Communicate with other employees in a private and respectful manner.27. Work and be proficient with certain computer systems designated by the company.

Supervisory ResponsibilityThis position has direct supervisor responsibilities for Geriatric Nurse Aides and other personnelassigned.

Work EnvironmentThis job operates in a health care setting. This role requires regular walking to various locationsaround the facility while pulling or pushing carts. This position works mostly in the Resident careareas as well as in occasional outside weather conditions. This role may also come into contactwith patients who may have contagious illnesses.

Physical Demandswhile performing the duties of this job, the employee is regularly required to talk and hear. Thisposition is very active and requires standing, walking, bending, kneeling, and stooping on a regularbasis. The employee must frequently lift and/or move objects and patients weighing over 75pounds.

Specific vision abilities required by this job include close vision, distance vision, color vision,peripheral vision, depth perception and ability to adjust focus.

Position Type and Expected Hours of WorkThis position is part of a health care facility that is open 24 hours a day, 365 days a year. Rotatingshifts as well as holiday and work during inclement weather will be required.

TravelNo travel is expected for this position.

Required Education and ExperienceHigh school diploma or equivalent.

Graduate from and accredited nursing program.

Current State License as a Licensed Practical Nurse or ed Nurse.

C.P.R. CertifiedPreferred Education and ExperienceOne year experience as a R.N. or L.P.N. in a long-term care setting.

One year experience as a C.N.A.Additional Eligibility QualificationsKnowledge of DPH regulations.
