Job Description
Busy 9 practitioner family practice has an immediate need for a customer service oriented care manager for a full time position. This position is available for a Registered Nurse only. Hours are Monday through Friday, 8:00 to 4:30 pm. Duties will include, but are not limited to the following: Care coordination Transitions of care management Chronic condition care management Communicating medical information between patient and physician Medication reconciliation Providing lab/test results to patients via letters, portal communication and telephone Handling triaged phone calls This is a 100% desk position. The right candidate will be able to create a friendly relationship with the patients and be able to communicate all medical questions and concerns as directed by the physician. Patients must remain the center of focus. Candidate should have computer skills and be prepared to deliver 5 star customer service. Candidate should be a consummate organizer, a self-starter, focused on the needs of patients, providers and colleagues. The appropriate candidate will have the ability to interact with tact, diplomacy and intelligence in a busy environment. We are seeking a true team player. Any newly hired Care Manager will be required to complete complex care management training through the Michigan Institute for Care Management and Transformation. This will be made available to you as part of your employment. Practice currently cares for 150-200 patients per day. Salary will be commensurate with state and national benchmark data based on experience and credentials.
