Show-Me Community Services is a family-owned organization providing person-centered services to adults with intellectual and developmental disabilities throughout Boone County. We are currently seeking a compassionate, dependable, and motivated ISL Home Supervisor to lead one of our Medical Home teams serving individuals with more complex medical needs. As the Home Supervisor, you will lead a team that delivers compassionate, person-centered supports while ensuring each individual's medical, behavioral, and daily living needs are met with dignity, respect, and professionalism. About the Role As an ISL Medical Home Supervisor, you will oversee the daily operations of the home while providing leadership and support to Direct Support Professionals (DSPs). This is a hands-on leadership role that combines staff supervision, training, scheduling support, documentation oversight, and direct care. You will play a vital role in maintaining quality services, supporting staff development, and ensuring compliance with company policies and Missouri Department of Mental Health standards. Essential Responsibilities Supervise, coach, and support DSP staff within the Medical Home Lead by example through professionalism, teamwork, and positive communication Ensure the health, safety, medical well-being, and quality of life of individuals supported Assist with staff training, mentoring, and performance coaching Monitor implementation of Individual Support Plans (ISPs), nursing recommendations, behavioral supports, and medical care plans Ensure documentation, medication records, and required reports are completed accurately and on time Maintain a clean, safe, and organized home environment Assist individuals with personal care, hygiene, mobility, transfers, meal preparation, medication administration, and other activities of daily living Coordinate transportation to medical appointments, Day Habilitation, employment, therapies, and community activities Communicate effectively with nursing staff, physicians, pharmacies, guardians, leadership, and interdisciplinary team members Assist with onboarding and ongoing staff development Must have availability to cover any shift when needed- days, evenings, overnights, weekends, and holidays Participate in an on-call rotation Maintain flexibility to rotate shifts as needed to support the home Qualifications Must be at least 18 years old High school diploma or equivalent required Valid driver's license, reliable transportation, and current auto insurance required Must pass a background screening or obtain an approved Good Cause Waiver Previous caregiving experience required; supervisory experience preferred Experience supporting individuals with medical needs, developmental disabilities, or nursing-related care is highly preferred Strong leadership, organization, communication, and problem-solving skills Ability to work flexible schedules and participate in an on-call rotation Ability to attend orientation and required certifications during normal business hours Paid Training & Certifications Training includes: Level 1 Medication Aide (L1MA) CPR & First Aid Positive Behavior Support (PBS) Why Join Show-Me Community Services?
