Full Job Description Nursing Facility in Concord, NH looking for Unit Manager Come join one of the BEST places around and make a difference in the lives of so many! The primary purpose of your job is to assume responsibility for and supervise all nursing care provided to residents Oversees nursing documentation on admission, throughout the resident's stay, and through discharge. Delegates responsibility to nursing staff and supervises the performance of their duties. Responsible for administering and/or delegating the administration of medication and treatment for the Unit. Completes/ensures completion of all admission, transfer, discharge documentation and communication in accordance with established procedures. Ensures that discharge teaching for medications, diet, activity level, etc., is completed and documented as required. Consults with physicians regarding the medical needs and changes in residents' conditions. Recognizes individual resident care needs and ensures the development of care plans tailored to meet these needs. Works closely and effectively with SDC, ICN, and MDS Coordinator. Responsible for the maintenance of records accounting for controlled drugs and inform DNS of discrepancies. Monitors Medicare charting as well as other documentation requirements of licensed and non-licensed staff. Attends or delegates attendance to weekly Medicare review meetings, daily nursing meeting, skilled admission meetings, and resident care conferences. Communicates to DNS and other shifts all pertinent resident information to promote continuity of care. Ensures that resident and family teaching is completed and documented where indicated. Communicates to DNS in-service education needs of staff and assists with training as necessary. Delegates the maintenance and cleaning of durable medical equipment. •
ON CALL ROTATION REQUIRED
