Job Description

RN Clinical Coordinator, Behavioral Health Position Summary The RN Clinical Coordination HBH serves as a leader and liaison between nursing staff, interdisciplinary team members and unit leadership. This position is responsible for efficient transition and continuity of care for behavioral health patients throughout the HBH continuum to ensure the delivery of safe, patient-centered, and evidence-based care. The Clinical Coordinator assures the optimization of patient flow through the emergency department, inpatient psychiatric unit, the medical floors, outside facilities in addition to referrals to outpatient BH treatment. This position provides direct patient care, supervision and clinical guidance to nursing staff, promotes adherence to regulatory standards and organizational policies. The Clinical Coordinator supports staffing coordination, onboarding and mentoring of new staff, and quality improvement initiatives. Primary Position Responsibilities Coordinate care across the continuum for Behavioral Health patients. Facilitates and Coordinates appropriate levels of care. Functions as a liaison between the ED, ES, OPS, 2Central and the Medical Floors. Demonstrates effective resource management and inter-professional Collaboration. Participates in staff evaluation and education requirements and acts as an experienced role model and support for all staff members. Maintains the standards of TJC, DOH, OSHA, EMTALA and specialty recommendations. Ensures quality patient care and improves patient care processes and outcomes. Facilitate and Coordinate Total Care Adheres to documentation guidelines and regulations Maintain Professional Development Other duties as assigned
Qualifications Minimum Education :
Required:
All hires after July 2012 are required to have a Bachelor's Degree in nursing OR if Associates Degree or Diploma, RN must provide proof of enrollment in a BSN program prior to hire, be continuously enrolled and complete within 5 years of hire date or internal RN promotion date.
Preferred:
None Minimum Years of Experience (Amount, Type and Variation) :
Required:
3 years Inpatient Behavioral Health Nursing experience
Preferred:
Prior supervisory experience is highly desirable.
License, Registry or Certification :
Required:
Current NJ RN licensure and American Heart Association CPR certification,
Violence Prevention Preferred:
Psychiatric/Mental Health Nurse certification or other certification in specialty Knowledge, Skills and/or
Abilities :
