A Nurse Manager manages nurses working in a clinical unit at a hospital or health care facility. Provides clinical experience as a registered nurse and supervises nurses that provide direct patient care. Creates and manages staff schedules and ensures that hospital policies and procedures are followed. Manages budget and recordkeeping for the unit.
RN Care Manager MUST be an RN and have at least 3 years of experience - this position rotates between our Administrative office as well as the Primary Care offices. Will be a M-F schedule - normal business hours. Salary will be determined by experience , licensure and overall qualificiations. Job Description The Case Manager supports high-risk patients by coordinating care across inpatient, outpatient, and post-acute settings. This role partners with Primary Care Physicians and the interdisciplinary team to ensure smooth care transitions, improved outcomes, patient satisfaction, and efficient use of resources. Core responsibilities include assessment, care planning, education, coordination, and documentation. Key Responsibilities Assessment & Planning Identify and assess high-risk patients. Develop individualized care plans with patients, families, and providers. Monitor progress and adjust plans as needed. Care Coordination & Transitions Support patients through hospital, home, and post-acute transitions. Educate patients and caregivers using evidence-based methods. Address barriers to self-management, including through home visits. Connect patients with internal programs and community resources. Collaboration & Teamwork Work closely with providers and staff across care settings. Serve as a consistent point of contact for patients and families. Act as a resource and partner to the healthcare team, fostering strong working relationships. Documentation & Quality Document assessments, plans, and interventions accurately. Apply best practices, regulatory standards, and quality measures to daily practice. Qualifications Current Ohio RN license (BSN preferred). 3+ years nursing experience; 1+ year case management preferred. Strong communication and teamwork skills. Ability to work independently and collaboratively. Flexible scheduling as needed. Physical Demands 1. Requires sitting for extended periods of time. 2. Some bending stretching, lifting and stooping may be required. 3. Requires manual dexterity sufficient enough to operate office equipment including but not limited to telephone, keyboard, copier and fax machine. 4. Requires normal range of hearing and vision. 5. Requires the ability to communicate with patients, office personnel and the community. Must comply with all OSHA and HIPAA regulations
