Yamhill County Correctional Facility Compensation:
$38.00 - $44.00 / hour Description We are looking for a MAT Coordinator to join our team and provide quality care to our patients at the Yamhill County Correctional Facility. This correctional facility houses both male and female inmates and provides the following services: medical and dental care, a commissary, social and educational programs, as well as drug and alcohol and psychological counseling.
Job Details:
Monday-Friday, 40 hours per week Onsite presence required; occasional flexibility for training or coordination meetings. Competitive pay based on licensure (RN vs. LPN) and experience. Comprehensive benefits package (medical, dental, vision, 401k, PTO).
Key Responsibilities Program Coordination:
Oversee daily operations of the MAT program including scheduling, medication administration, documentation, and coordination of care. Serve as the primary liaison between medical staff, mental health providers, custody staff, and community partners to ensure a seamless MAT process. Assist with program expansion, audits, and compliance reviews to meet NCCHC and Oregon state requirements.
Patient Care & Clinical Duties:
Administer MAT medications (e.g., buprenorphine, methadone, naltrexone) in accordance with provider orders and facility protocols. Conduct intake screenings and assessments for MAT eligibility in collaboration with providers. Provide education to AICs on MAT, medication adherence, and recovery support. Monitor and document patient response to medications, side effects, and treatment outcomes.
Discharge Planning & Continuity of Care:
Coordinate discharge planning for MAT participants to ensure seamless continuity of care upon release. Establish and maintain referral relationships with community MAT providers and recovery programs. Ensure proper release-of-information and documentation are completed for continuity of treatment.
Training & Oversight:
Work alongside nursing and custody staff to ensure safe and effective medication delivery practices. Train nursing staff in MAT-related protocols and support new staff in understanding MAT workflows. Collaborate with the medical provider and site leadership to evaluate and improve MAT program delivery. Skills, Knowledge and Expertise Strong organizational skills with the ability to manage multiple priorities. Excellent communication and teamwork skills to coordinate across medical, mental health, and custody staff. Ability to provide compassionate, nonjudgmental care to individuals with substance use disorders. Competence with EMR systems (experience with CorEMR preferred).
Qualifications Education & Licensure:
Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current, unrestricted Oregon license. Current CPR/BLS certification required.
Experience:
Minimum of 1-2 years of clinical nursing experience (REQUIRED) Experience in correctional healthcare, addiction treatment, or psychiatric/behavioral health (REQUIRED) Familiarity with NCCHC, HIPAA, and Oregon Board of Nursing standards of practice.
