Position Overview We are seeking compassionate, experienced professionals to join our growing Care Management team.

Occupational Therapists (OTs) Registered Nurses (RNs) Social Workers Daily Money Managers Other qualified professionals with care management or nursing experience As a Care Manager, you will work directly with clients and families to assess needs, develop care plans, coordinate services, and provide ongoing support and advocacy. Flexible Growth Opportunity This position begins on a part-time basis , allowing you to build relationships with clients and develop your caseload. As your caseload grows, you will have the opportunity to transition into a full-time role , creating a flexible pathway for professionals seeking long-term career growth with our organization. Responsibilities Conduct client assessments and identify needs Develop and implement individualized care plans Coordinate healthcare, social, financial, and community resources Advocate for clients and families Monitor client well-being and adjust care plans as needed Maintain accurate documentation and records Communicate effectively with clients, families, providers, and community partners

Qualifications Preferred Background:

Licensed Occupational Therapist (OT) Registered Nurse (RN) Licensed Social Worker Daily Money Manager experience or certification Care Management, Case Management, Geriatric Care Management, or related experience 10+ years experience preferred

Additional Requirements:
