Full Job Description Position Summary:
A Caring Connection is seeking an experienced Registered Nurse (RN) to join our team as a Clinical Care Coordinator. This hybrid position is responsible for supporting the day-to-day clinical operations of the agency and serving as a resource for nurses, supervisors, clients, and families. The Clinical Care Coordinator plays a key role in ensuring quality patient care, staff support, regulatory compliance, and operational efficiency. The ideal candidate is organized, clinically experienced, comfortable working independently, and interested in supporting both field staff and office-based clinical operations. While much of the work can be performed remotely, occasional office visits and field activities will be required for training, onboarding, and clinical support functions. Key Responsibilities Support day-to-day clinical operations of the agency Serve as a clinical resource for nurses, supervisors, clients, and families Conduct occasional field visits to support existing clients and nursing staff Assist with nurse onboarding, orientation, training, and competency evaluations Manage supervisor staffing, scheduling, and management activities Review and assist with clinical documentation and compliance processes Assist with clinical follow-up, communication, and operational support as needed Respond to routine clinical questions during business hours Collaborate with the clinical leadership team to maintain quality standards and regulatory compliance Preferred Schedule Flexible schedule Primarily remote work Approximately 20-25 hours per week (depending on candidate availability) Ideal availability would include mid-morning through mid-afternoon coverage to provide consistent support to the clinical team Occasional office visits in Scotch Plains and field visits as needed Qualifications Active Registered Nurse (RN) license Strong clinical assessment and communication skills Home care, private duty nursing, pediatric nursing, case management, or supervisory experience preferred Ability to work independently and manage multiple priorities Strong organizational and documentation skills Experience with electronic medical records (EMR) systems preferred About the
