POSITION SUMMARY
In coordination with the Medical Director and Company leadership, works to expand our footprint within the medical communities we serve. Offers insight and training to our clinical team to bring evolving clinical practices into our operations. In coordination with the ADON, QA's clinical documentation - paying particular attention to the quality and completeness of documentation ensuring it meets quality and compliance standards, as well as answers (and avoids) any clinical utilization review issues. Regularly audits documentation to clinical orders ensuring the full scope of services are completely documented. Oversees patient admissions utilizing EHR system ensuring timeliness and completeness; H&P is solid, admissions documentation is solid, reporting of new admission, HIS, etc., is timely. Works closely with billing team to ensure related documentation is correct and meets timely billing criteria. Ensures accurate scheduling of visits, HOPE visits, Recertification and F2F visits, then ensures these visits and physician narratives are sent and signed in required timeframes. Continually monitors training needs. Develops and delivers programs (individually or group) to ensure quality patient care and compliance. Works with ADON on the development of the monthly on-call scheduling for the service area. While maintaining a hospice case load, assists in the training and ongoing evaluation of RN's and care givers. Along with ADON and Exec Dir, works to solve daily operational issues. Liases with Patient Liaison's and ancillary team to facilitate smooth admissions. Works in concert with the Company leadership and sales to develop strong relationships with referral sources, including conducting meetings and presentations. Ensures the professional provision of superior patient care and support to patients family through assigned staff. Evaluates and mentors clinical team. Leads bi-weekly IDT Meetings, and follows up with needed issues. Participate as a integral part of the True Hearts and Hands Hospice Team offering guidance and insight into care delivery, compliance and operations for ongoing improvement and continuing reputation of quality and integrity.Requirements Include:
Current Texas State license as a Registered Nurse. 6+ years of hospice experience as an RNCM.. 3+ years of Hospice supervisory experience in roles of increasing responsibility. Ability to work in a field setting and proven ability to make sound nursing judgments Ability to assess patient needs and formulate individualized patient care plans to meet those needs; directly and indirectly. Effective communication skills Familiarity with electronic admission and charting. Ability to quickly master various applications of the EMR.Job Types:
